Requesting a Web Filter Exception:

Web filter exceptions (unblocking websites) will be processed through the completion of a Google Form.  The form can be completed by clicking HERE.  Upon clicking the submit button at the bottom of the form, a ticket will automatically be generated for you.  There is no need for you to submit a separate ticket after you have submitted the request form.

 

Once the form is submitted, the website will be reviewed by the Technology Services department to ensure that it is a safe website that complies with all school district policies, and local, state, and federal laws and regulations.  Once the site has been deemed safe, the request will be sent on the building principal.  The building principal will make the decision to either unblock the site, or to leave the site blocked.  Either way, you will be notified of the decision.

 

Door entry card modifications will be processed through the completion of a Google Form.  The form can be completed by clicking HERE.  Upon clicking the submit button at the bottom of the form, a ticket will automatically be generated for you.  There is no need for you to submit a separate ticket after you have submitted the request form.

 

This form will be used to request a new card, a replacement of an existing card, modification of an existing card, or deactivation of an existing card.  Please note that on the form there are separate sections  depending on your type of request.

 

iPad app requests will be processed through the completion of a Google Form.  The form can be completed by clicking HERE.  Upon clicking the submit button at the bottom of the form, a ticket will automatically be generated for you.  There is no need for you to submit a separate ticket after you have submitted the request form.

 

Technology purchase requests will be processed through the completion of a Google Form.  The form can be completed by clicking HERE.  Upon clicking the submit button at the bottom of the form, a ticket will automatically be generated for you.  There is no need for you to submit a separate ticket after you have submitted the request form.

 

Please note, requesting the purchase of technology does not guarantee that the technology will be purchased.  Requests will be reviewed by the Technology Committee.  ALL purchases of technology, even if being purchased through your own budget, a program budget, etc. must be requested using this form.  It is imperative that the technology that we purchase meet the standards of the district, integrate with our existing systems, and be the most cost effective solution available.
