Facilities Request Forms and Work Order Forms


Use of Facilities Request Forms, found under the "Forms" link must be filled out and turned into the corresponding contact person or completed and emailed to the corresponding contact person appearing below two weeks prior to the date of your event:


Mr. Mike Reilly, Athletic Director, for use of the elementary gym, multipurpose room, and high school gym.

Mrs. Michelle Dolges, High School Principal, for the use of all high school facilities except the high school gym.

Mr. David Grimaldi, Elementary Prinicipal, for the use of all elementary facilities except the elementary gym and multipurpose room.


Please also refer to the Use of Facilities Adminstrative Regulations appearing under the "Forms" link prior to submitting your request to be certain you have completed your request form correctly.


Work Order Forms must be filled out and turned into Mark Dalton via email: or printed and hand delivered.  The work order forms can be downloaded by clicking here or going to the "Forms" link and choosing Work Order Form.
